2. Name (Identical to ISACA record)
3. Email (Identical to ISACA record)

* If you provided an invalid ISACA ID, you will be removed from the registration list.

• Chapter will upload your CPE hour to ISACA HQ.
• Chapter will keep you informed once CPE recording to ISACA HQ is completed.
• Email notification from the chapter will be sent within a week after the event.
• Apply your CPE hour by login at www.isaca.org/myisaca

• Please be reminded that for CPE hour entitlement, you have to be present for the event for at least 50 minutes.

• The attendance registration will be closed about 10 minutes before the seminar starts. After that, late participants could still join the seminar, but will not gain the CPE hour due to the above CPE entitlement rules.
